The headlines read “Pharmacy would rather remove Yaz’s anti-impotence medication” and “Will it stop a drug that was once branded Viagra”. With more and more women reporting side effects from their tablets, it’s no surprise that there is a strong inclination to lump them with the side effects associated with Viagra and other oral hormonal drugs.
However, the headlines have a another solution: The removal of Yaz from the sex-eds routine.
In fact, the UK’s Medicines and Healthcare products Agency (MHRA) previously recommended Yaz (terazosin HCl) as the only medication that could be combined with Viagra/sildenafil to treat impotence. However, this recommendation was later withdrawn.
This decision has been made after extensive scientific studies and a thorough review of the available data. The UK MHRA has also weighed in on whether to reclassified Yaz (terazosin HCl) as an antidepressant.
However, there is no indication that Yaz has a similar effect to Viagra/sildenafil. The only other UK medication to be considered for this classification is Zydus Cadila (dapoxetine). However, Cadila is only available on prescription and is withdrawn from the market following criticism that it was unlicensed and unreliable.
In 2019, Yaz was approved for the treatment of impotence in 152,849 women (boys: 62,152, females and girls-": 28,737, non-boys and 12,093, girls).
At the time, the drug was listed as having a low risk of causing sexual dysfunction, with the risk in the first year only. A similar review by the Royal College of Obstetricians and Gynaecologists (RCCOG) found that women who used Yaz reported lower rates of sexual dysfunction than women in their forties or fifties (boys: 3.8% and girls: 1.8%).
However, it is the side effects from Yaz that have the most to lose. The most common side effects were headaches, dizziness and indigestion, with other side effects include breast tenderness and enlargement. These side effects were previously rare and included a very low incidence of priapism (prolonged and painful erection) and an unfavourable patient experience with the penis.
The FDA has also previously criticised Cadila for the poor safety profile and for the potential for side effects to occur in other formulations. However, Cadila has since been listed as a ‘Top 100 Contraindications of Yaz’ by the UK’s PPCO. PPCO has also released a list of the top ten contraindications of Yaz (from the US FDA’s list of the top ten prescription medications).
The removal of Yaz from the sex-eds routine is not the only option.
With the removal of Yaz, the UK’s MHRA recommends that other oral hormonal drugs, including PDE5 inhibitors (dapoxetine and Venlafaxine), be approved for the treatment of impotence, rather than Yaz (terazosin HCl).
Another way in which the move could be beneficial is if Yaz is removed from the sex-eds routine.
